The Anticoagulation Clinic at Firelands Regional Medical Center was established to provide safe and effective management of anticoagulation therapy for both ambulatory and inpatients. The clinic is staffed by clinical pharmacists with special training in anticoagulation therapy and assists physicians in the management of patients placed on anticoagulation drug therapy.

Why is an Anticoagulation Clinic needed?
Anticoagulant medications (Warfarin, Coumadin, Lovenox, Fragmin, Arixtra and others) can be dangerous medications if not closely monitored. The Anticoagulation Clinic provides patients with rapid and efficient monitoring and management of these medications to achieve the best possible outcome while minimizing the risk of any adverse events.

What is required to be enrolled in the anticoagulation clinic?
Your physician must complete and send a referral form to the Anticoagulation Clinic. This physician must be someone whom you intend to continue to visit at least once a year. You must be willing and able to come in for a 15 minute appointment with our clinic pharmacists at least once a month, but more often if necessary.

What is the procedure for follow-up in the anticoagulation clinic?
After the clinic receives a completed referral form, you will be contacted by the Anticoagulation Clinic to discuss your therapy and to arrange your first appointment. At your scheduled appointments, the pharmacist may check your INR (a measure of how quickly your blood is clotting) by using a drop of blood from your finger (the result will be available immediately). The pharmacist will ask some questions about your health, diet and medications and determine the most appropriate dosage and frequency for your anticoagulant medication(s). Your first appointment will take approximately 45 minutes as an educational presentation will be provided in addition to your routine INR test.

The Anticoagulation Clinic is conveniently located on the second floor of the Firelands Regional Medical Center Main Campus, 1111 Hayes Avenue. The Anticoagulation Clinic hours are as follows:

Mondays, Tuesdays, Thursdays, and Fridays
7:00 am - 3:00 pm

Wednesdays
8:00 am - 4:00 pm
