Consent for Emergency Treatment
Parents can authorize a relative or specific caretaker to seek medical help for their child in case of a medical emergency. Or, they can grant treatment authorization directly to Firelands Regional Medical Center's Emergency Department medical staff.

If you would like to have treatment authorization on file at the Firelands Emergency Department, simply print out the Consent for Emergency Treatment form, fill it out, and mail it to the address at the bottom of the form. This consent form will be kept on file for one year.
