Heart Failure Clinic

The Heart Failure clinic at Firelands Regional Medical Center provides patients with tools to help monitor heart failure symptoms and manage their medications to reduce the risk of hospitalization due to heart failure. A clinical pharmacist and a nurse with specialized training in heart failure will help educate you on your condition and the improvements you can gain through diet, exercise and medication management.

How do I enroll in the Heart Failure Clinic?

Any patient with a diagnosis of heart failure may enroll with a physician referral. This physician must be someone whom you intend to continue to visit at least once a year.

Once the clinic receives a referral form, you will be contacted by the heart failure clinic to discuss your therapy and to arrange your first appointment.

The initial appointment takes approximately two hours and includes a detailed physical assessment with our clinic staff. Shorter follow-up visits are necessary for monitoring, education, and interventions. Remote monitoring is available for some patients.

Where is the Heart Failure Clinic located?

The Heart Failure Clinic is at the Firelands Center for Coordinated Care at the Hayes/Tyler corner of the Firelands Regional Medical Center main campus Medical Arts Building, 1221 Hayes Ave., Suite F. The heart failure clinic hours are:

  • Monday 8:15 a.m. to 3:15 p.m.
  • Tuesday 8:15 a.m. to 3:15 p.m.
  • Wednesday 8:15 a.m. to 3:15 p.m.
  • Thursday 7:15 a.m. to 2:15 p.m.
