Heart Failure Clinic
The heart failure clinic at Firelands Regional Medical Center was established to assist physicians in providing safe and effective management of ambulatory heart failure patients. The clinic is staffed by clinical pharmacists and a registered nurse with specialized training in heart failure.
Why is a heart failure clinic needed?
Heart failure is a disease requiring long-term management of medications and symptoms. If not monitored closely, patients may have recurring signs and symptoms interfering with their quality of life and requiring frequent readmissions to the hospital. The heart failure clinic provides patients with rapid and efficient monitoring and management to achieve the best possible outcome while minimizing the risk of hospitalization.
How do I enroll in the heart failure clinic?
Any patient with a diagnosis of heart failure may be enrolled. Your physician must complete and send a referral form to the heart failure clinic. This physician must be someone whom you intend to continue to visit at least once a year. You must be willing and able to come in for an initial two hour appointment with our clinic staff, followed by shorter repeat visits as necessary for monitoring, education, and interventions.
What is the procedure for follow-up in the heart failure clinic?
After the clinic receives a completed referral form, you will be contacted by the heart failure clinic to discuss your therapy and to arrange your first appointment. During your initial appointment, the nurse will ask questions about your health, diet and medications. A detailed physical assessment will be performed. In addition, we will provide education and numerous tools to enable your independence, improve your quality of life and reduce the potential for future hospitalization. Remote monitoring is available for some patient groups.

The heart failure clinic is conveniently located at the Firelands Center for Coordinated Care at the Hayes/Tyler corner of the Firelands Regional Medical Center main campus Medical Arts Building, 1221 Hayes Ave., Suite F. The heart failure clinic hours are:
- Monday 8:15 a.m. to 3:15 p.m.
- Tuesday 8:15 a.m. to 3:15 p.m.
- Wednesday 8:15 a.m. to 3:15 p.m.
- Thursday 7:15 a.m. to 2:15 p.m.
