Rules and Regulations: The Foundation for Firelands Golf Cart Raffle is sponsored by Janotta & Herner to benefit patient care at Firelands Health. This raffle is conducted by The Foundation for Firelands, a nonprofit organization under the laws of Ohio. Void where prohibited by law. By purchasing a raffle ticket, you agree to be bound by these rules: 1. All entrants must be at least 18 years old and possess a valid driver’s license. 2. The purchase price for one raffle ticket is $100.00 each and is not tax-deductible. A maximum of 600 tickets will be sold. 3. All entry purchases are final and non-refundable. 4. Entrant will retain the original printed raffle ticket and the stub will be entered into the drawing and retained by The Foundation for Firelands (TFFF). 5. The winner of the raffle will be determined by random drawing which will take place on Monday, August 3rd, 2026 at The “Caddyshack” Open Golf Tournament before the 11:00 am shotgun start. 6. Winner need not be present to win and will be contacted via the phone number and email listed on their raffle ticket. 7. Winner agrees to comply with all IRS regulations regarding raffle winnings. Winner agrees to provide TFFF with their social security number, verify their identity, and sign all required IRS documents before claiming their prize. 8. TFFF specifically disclaims all expressed or implied warranties, including without limitations warranties of merchantability. TFFF is not responsible for any losses or injuries suffered as a result of the raffle prize. 9. The winner of the raffle must claim the prize within 30 days of the drawing and must transport the prize to their designated location. If the winner does not claim the prize within 30 days of the drawing, the prize will be forfeited. A new winner will be drawn. 10. Winner is responsible for all taxes, tags, registration and tax forms per the winner’s state and the state of Ohio. 11. Purchaser of raffle ticket must legibly write their name, address, email, and phone number on each ticket purchased. If more than one name is stated on a ticket entry, the first listed name shall be the entrant. It is the sole responsibility of the entrant to allocate any such prize among multiple participants in a multiple party entry. 12. Purchasers of raffle tickets release TFFF and Firelands Health, its directors, officers, members and agents from any and all liability, loss, or claim arising out of any raffle prize. 13. Prize is not redeemable for cash. 14. Winner agrees to allow TFFF to publish their name and photographs on promotional material and social media.
