How can I get registered to use the Health eChart patient portal?
You may only register for a patient portal account after your first visit at Firelands Health The Bellevue Hospital.
- Upon patient registration, you will be asked if you would like to register for the patient portal. Please supply your correct email address during the registration process. A secure invitation will be sent to you via email. Your email invitation to the Health eChart will enable you to log in and create your own username and password.

Access on your smartphone
Did you know the patient portal also comes with a mobile app? It's called Meditech MHealth and you can download it on your device through the Apple Store and Google Play.
Access your child's portal
If a patient is a minor, authorization by the minor’s parent or legal guardian is required to grant proxy access.
In order to comply with U.S. and Ohio laws and regulations concerning the privacy of certain medical/mental health records of minor patients, The Bellevue Hospital does not provide Health eChart website access to the electronic health record of minor patients from the ages of 13 to 17 without consent. Spouse or caregiver access

Who do I contact for possible errors in my record?
Your information on the Health eChart patient portal comes directly from your electronic health record. If you find errors, please contact our Health Information Management department at records@bellevuehospital.com, or by calling 419-483-4040, ext. 4212, to correct any inaccurate information.
