Firelands Health The Bellevue Hospital offers comprehensive cardiac and pulmonary rehabilitation programs designed to promote recovery, prevent future complications and improve overall quality of life.
Our multidisciplinary team of doctors, nurses and dietitians works together to support you. Each participant receives an individualized plan tailored to their physical condition and emotional well-being, ensuring safe, effective progress toward recovery.
Cardiac rehabilitation
Cardiac rehabilitation helps patients recover following heart attacks, bypass surgery, angioplasty or other heart-related conditions. The program focuses on improving physical fitness, encouraging a heart-healthy diet, managing cardiovascular risk factors, following medical treatment plans and supporting emotional well-being and social engagement.
The program includes two phases:

Phase II: A program for outpatients focusing on exercise and education after a heart event or procedure. Most insurance plans cover phase II with a qualifying diagnosis.

Phase III: A maintenance phase where patients continue healthy habits through exercise and diet, for a low out-of-pocket fee.
Pulmonary rehabilitation
Pulmonary rehabilitation supports individuals with chronic lung diseases, such as emphysema, chronic bronchitis, asthma, bronchiectasis and interstitial lung disease. The program combines exercise, education, breathing retraining, emotional support and nutrition counseling to help participants reach their highest level of physical function and independence.

Supervised exercise therapy for peripheral artery disease
Supervised exercise therapy is for patients diagnosed with peripheral artery disease (PAD). This physician-referred program focuses on walking exercises that help participants increase endurance and reduce leg pain. Sessions take place three days a week for up to 12 weeks, lasting about one hour each and including intermittent rest breaks and education on nutrition, PAD risk factors and smoking cessation.
A referral is required to enroll.
