Physician order testing
Your physician may order lab tests during your visit. Firelands Health The Bellevue Hospital provides convenient walk-in testing.
Appointments are not required. For faster service, you may preregister by calling 419-483-4040, Ext. 0.
Direct access testing
Through our direct access testing program, the listed laboratory tests are available without a physician’s order at affordable prices. No insurance is accepted. You may pay with a credit or debit card.

Walk-in testing options include:
- Complete blood count with comprehensive metabolic and fasting lipid panel - $45 (Liver and kidney function studies, fasting blood sugar, cholesterol, HDL/LDL and triglyceride levels, along with a complete blood count.)
- Vitamin D - $35
- Hemoglobin A1C - $20 (A three-month report card on how well your blood sugars have been running. A test used to diagnose diabetes and/or to evaluate how well your treatment plan is working.)
- HCG - Pregnancy - $15 (Blood test, qualitative results only. No numerical value).
- Thyroid stimulating hormone (TSH) - $25
- Fasting lipid panel - $20
- PSA (prostate specific antigen) - $30
- Fecal occult - $25

COVID-19, RSV, influenza and strep testing
The Bellevue Hospital offers walk-in testing for respiratory illnesses, allowing individuals to order tests for COVID-19, influenza, RSV and strep throat at an affordable price without a physician’s order.
You may pay with a credit or debit card. Patients who prefer to process testing through insurance must have a physician’s order.
Appointments are not required, but preregistration is required.

- Rapid COVID-19 antigen test - $25
- Rapid Influenza A and B test - $40
- Rapid RSV test - $40
- Rapid strep throat test - $20
