The medication management clinic provides specialized care for patients taking anticoagulants or multiple prescriptions for various health conditions.
Anticoagulants are used to prevent and control the development of blood clots. Because these medications can be dangerous if not properly monitored, careful management is essential. The clinic helps patients safely manage their therapy while reducing the risk of complications. Examples of anticoagulants, also known as blood thinners, include Coumadin (warfarin), Xarelto, Pradaxa, Eliquis and Lovenox.
Patients who take anticoagulant medications or multiple prescriptions are eligible to participate in the medication management clinic. The program is designed to help patients, in collaboration with their physicians, maximize treatment effectiveness and avoid unwanted side effects.
Specially trained pharmacists meet with each enrolled patient to review medications, diet, recent health history and other relevant information. During the visit, a small drop of blood is collected via a fingerstick. The pharmacist uses this sample to determine whether adjustments are needed to the patient’s blood thinner medication. Results are shared immediately with the patient, and a report is sent to the referring physician.

A physician referral is required to be seen in the clinic. Once a referral is received, a pharmacist will contact the patient to schedule an appointment.
