We no longer require a Social Security number 

Firelands Corporate Health Center is proud to partner with enhanced SYSTOC services for your occupational health needs. We no longer require a Social Security number as a secondary identifier. Employers are encouraged to provide a birth date instead.

We’re Digital

Save time by planning ahead. Patients can now complete their history forms before arriving at the clinic, streamlining the check-in process. This service helps reduce wait times and ensures their information is seamlessly imported into our system, cutting down their in-clinic visit time.

When scheduling an appointment, please have the employee’s email address and birth date ready. Appointment-related emails will be sent from DoNotReply@PureSafety.com.

DOT Physicals

DOT Physicals are highly regulated. To ensure a smooth process, please provide the attached document to your drivers before their appointment.

Fingerprinting

Firelands CHC is a proud WebCheck© Vendor for fingerprinting services. In order to continue providing the highest level of fingerprinting services to our clients, while also remaining compliant with Webcheck© requirements, we want to connect with your company to verify and update fingerprinting information.

Webcheck© requirements are as follows:

  • All fingerprinting information must come from the client requesting the fingerprints. This includes the codes required for BCI, FBI or BCI/FBI as well as direct copy instructions and a mailing address for results.
  • BCI and FBI codes need to match, meaning the reason listed in the description should be similar.
  • The person being fingerprinted must have a valid ID
  • Minors must have their parent or legal guardian physically present with a validated photo ID to sign consent and witness the fingerprinting process.

After-Hours Drug Screening Authorizations

Each employee must have an authorization for service. If you have an employee who must use Firelands ED or FPG Urgent Care, please print and complete the form below and send it with your employee.
